Faculty of advanced training and professional retraining of Krasgma specialists. Postgraduate education Chmykhalo Natalia Borisovna

The strategic objective is to meet the needs of practical healthcare in the training of qualified personnel based on the following requirements for medical education: 1) compliance with the needs of healthcare in terms of quantitative and qualitative indicators of specialist training; 2) efficiency; 3) continuity; 4) competitiveness based on quality standards; 5) continuity.

According to the existing legislative framework, the main stages of postgraduate (postgraduate) education are internship, clinical residency, advanced training and professional retraining, and internship of specialists.

Internship, which represents the actual medical internship, is a mandatory stage for graduates of medical universities who want to obtain the right to independent medical practice.

In general, over the past 5 years, the number of specialists who completed their internship decreased by 3.4%, mainly due to a decrease in the number of intern doctors trained at medical institutions in the Krasnoyarsk Territory (Table 6).

Over the same period of time, the number of specialists who completed clinical internships increased by 25.7%.

As of January 1, 2007, 151 people entered clinical residency on a budgetary and extra-budgetary basis in 2006. The main share of those admitted to clinical residency are graduates of Krasnodar State Medical Academy this year (67.0%), doctors after internship training - 15.9%, specialists with more than one year of experience in practical healthcare - 13.3%.

46 specialists (45.0%) have contracts for subsequent employment in primary health care.

In 2006, to improve the quality of training of clinical residents, a system of department integration was used when training specialists in fundamental and related disciplines, as well as in the section “public health and healthcare”; A systematic survey was conducted on the effectiveness of teaching sections and disciplines.

To solve organizational problems identified during the analysis of primary specialization based on the results of 2005, in 2006: 1) a system for promoting the employment of “young” specialists was developed; 2) control was carried out over the quality of concluded contracts for the employment of specialists; 3) the formation of a system of primary specialization focused on primary health care began.

^ Advanced training (professional retraining) of specialists

Training and retraining of specialists with medical education at KrasSMA is carried out annually within the framework of the educational and production plan approved by the Ministry of Health and Social Development.

Over the past 5 years, the plan for training specialists in advanced training cycles has been increased by an average of 19.6% (Table 9). At the same time, until 2005, the plan was exceeded, reaching 7.9% (2004). In 2006, according to preliminary data, the plan for training specialists in absolute terms was fulfilled by 93.0%.

^ Promotion of employment for “young” specialists

The negative socio-economic processes that took place in the 90s of the last century undoubtedly affected the state of the human resources potential of the healthcare industry. Outflow of specialists from state and municipal healthcare institutions, primarily the territories of the Krasnoyarsk Territory, occurred for the following main reasons:

  1. abolition of state distribution of graduates;

  2. non-payment and low level of wages with fairly high responsibility (psychological, legal);

  3. low level of social and living conditions in the territories of the region;

  4. division of powers for responsibility for the state of the healthcare system between state and municipal executive authorities;

  5. the emergence of a non-state health sector;

  6. reduction of the state (federal) order for the training of specialists with higher medical education;

  7. “artificial” preservation of vacant positions for internal part-time work and the reluctance of the heads of medical institutions to hire young specialists.
Thus, by the beginning of 2006, the shortage of doctors of all specialties in the region, as already indicated, amounted to almost 4,000 people with a high percentage of part-time jobs (1.9). Against this background, there remains a low percentage of staffing with specialists providing specialized types of medical services (psychiatrist - 63.3%, phthisiatrician -58.0%, pathologist - 37.0%, etc.)

In the process of implementing a priority national project in the healthcare sector, it was possible to improve the staffing level of doctors in primary health care (local therapists - 76.2%, local pediatricians - 91.9%).

In 2005, the graduation rate of specialists from the medical, pediatric and dental faculties on a budgetary and extra-budgetary basis amounted to 474 people, including 133 (28.1%) on extra-budgetary ones.

^ The following agreements have been concluded for primary specialization:

  1. with medical institutions located in the Krasnoyarsk Territory - 301 (63.5%) graduates;

  2. with medical institutions of the Republic of Khakassia, the Republic of Tyva, Evenki and Taimyr Autonomous Okrugs - 92 (19.4%) graduates;

  3. 35 (74%) graduates were trained on a commercial basis.
Based on the results of the employment analysis, through written requests to medical institutions of the Krasnoyarsk Territory, specialists who have completed primary specialization, it was established that 200 doctors (66.4%) were employed, including:

  1. 119 specialists in municipal healthcare institutions of the region;

  2. 51 - in regional state health care institutions;

  3. 30 – in departmental health care institutions.
45 specialists continue their studies (clinical residency).

56 (18.6%) specialists are not employed, of which 15 are on maternity leave. That is, 41 people (13.6%) find their application in non-state and municipal healthcare systems.

37 young specialists were employed in primary health care, including after activities carried out in the implementation of a priority national project in the field of health care.

However, in general, I would like to emphasize that 81,4% Doctors graduated in 2005 must be considered employed.

^ During the assessment of employment results, the following problems were identified that required solutions:

1) the difficulties of non-issuance of documents on completion of internship, residency upon successful passing of exams related to existing legislation;

2) imperfection of legislation regarding the fulfillment of obligations under concluded contracts for a three-year period of work;

3) the presence of fictitious internship contracts;

4) lack of measures on the part of the heads of healthcare institutions to return “young” specialists, in accordance with the concluded agreements.

Thus, according to the concluded agreements with City Clinical Hospital No. 20 of Krasnoyarsk - 10, State Clinical Hospital No. 14 of Krasnoyarsk - 6, Kuraginsk Central District Hospital - 4, Kansk Central City Hospital - 4, etc. graduates are not employed in healthcare institutions - no measures have been taken by the chief doctors to the return of young specialists, in accordance with signed agreements.

Completed targeted training in 2006 55 graduates KrasGMA , including 26 – Faculty of Medicine, 20 – Faculty of Pediatrics, 9 – Faculty of Dentistry.

Agreements on undergoing primary postgraduate training with municipal health care institutions of the territories from which they were sent were concluded by 52 (94.5%) people, including:

  • 24 (92.3%) graduates of the Faculty of Medicine;

  • 19 (95.0%) graduates of the Faculty of Pediatrics;

  • 9 (100%) graduates majored in dentistry.
To date, 3 graduates of Krasnodar State Medical Academy (2 people from the Faculty of Medicine, 1 person from the Faculty of Pediatrics) have not concluded an agreement on undergoing primary postgraduate training, which is 5.5% of the total number of graduates who graduated from the university according to the target enrollment.

It should be noted that the formation at KrasSMA of a system of assistance in the employment of specialists started in January of this year. This work is entrusted to the department of assistance in employment of “young” specialists of the Institute of Postgraduate Education of Krasnoyarsk State Medical Academy.

Over the past period of time, the following activities have been carried out.

In March 2006, together with the Health and Drug Supply Agency of the Administration of the Krasnoyarsk Territory, the distribution of graduates of 2006 was carried out, according to the results of which all, with the exception of 3), studying at KrasSMA for targeted contract training, had agreements with the relevant medical institutions for subsequent employment.

During the 2005-2006 academic year, special attention was paid to working with clinical interns and clinical interns.

KrasSMA specialists held a number of seminars, round tables with interns and clinical residents, primarily those studying in the specialties of “therapy”, “pediatrics”, “general medical practice (family medicine)”, at which issues of implementing a priority national project in the field of healthcare, including improving the work and strengthening primary healthcare.

To assess the employment of specialists who graduated from clinical residency and internship in 2006, the State Educational Institution of Higher Professional Education of KrasSMA Roszdrav in August 2006 sent letters to the heads of medical institutions with a proposal to provide information on the actual employment of doctors.

In September-October of this year, meetings were held with 6th-year students of the Faculty of Medicine and Pediatrics, 5th-year students of the Faculty of Dentistry on issues of employment, orientation of future graduates to primary health care.

Since March 2006, work has begun to post information about vacancies in healthcare institutions of the region on the KrasSMA website. There are currently information available for 238 vacant positions. To improve this work, at the end of October, letters were sent to the heads of health authorities and institutions of the region inviting them to provide information on available vacancies. However, it is necessary to develop a system for continuous monitoring of the closure and opening of vacancies.

To assess the quality of specialist training, at the beginning of November this year, a questionnaire “Quality of specialist training in Krasnodar State Medical Academy” was sent to healthcare institutions in the region.

To organize events to introduce senior year students to medical institutions in the territories of the region, letters were sent to the heads of these institutions with proposals: to provide electronic resources of institutions for posting links on the KrasSMA website and posting on their sites advertising information intended for students (characteristics of institutions, types, volumes and profiles of medical care, social conditions, etc.).

During this academic year, a number of trips with graduates to the region are planned to present medical institutions, working conditions and social conditions. Already in December, a trip to Lesosibirsk was carried out. But there is a problem for carrying out such work - financial data of the events (transport costs, etc.).

Also, the KrasSMA budget does not provide for the costs of sending graduates for practical training in the region.

At the beginning of 2007, individual work is planned to promote the employment of specialists in problem areas and health care institutionswith the staffing indicator for full-time positions of local general practitioners:

below 70%: Bolshe-Uluyskaya Central District Hospital, Achinskaya Central District Hospital, Nazarovskaya Central District Hospital, Pirovskaya Central District Hospital, Sayanskaya Central District Hospital, Divnogorsk Central District Hospital, Ilanskaya Central District Hospital, GP No. 7 of Krasnoyarsk, GP No. 2 of Lesosibirsk - (9 institutions),

below 60%: Kuraginskaya Central District Hospital, Uzhurskaya Central District Hospital, Sharypovskaya Central District Hospital, State Enterprise No. 2 of Achinsk, State Enterprise of Minusinsk, State Enterprise No. 1 of Krasnoyarsk - (6 institutions),

below 50%: Kazachinskaya Central District Hospital, Kanskaya Central City Hospital, Motyginskaya Central District Hospital, Strelkovskaya City Hospital (Lesosibirsk), State Hospital No. 1 of Achinsk, City Hospital of Achinsk, local hospitals of the Nazarovsky district - (7 institutions),

with the indicator of staffing of regular positions of local pediatricians

below 70%: Achinskaya Central District Hospital, Taseevskaya Central District Hospital, local hospitals and outpatient clinics of the Nazarovsky and Sharypovsky districts (4 municipalities),

below 50%: Pirovskaya Central District Hospital - (1 institution).

In February 2006, it is planned to conduct a survey of students to assess their opinion on what factors (reasons) they rely on in their desire or reluctance to work in health care institutions in the territories of the region.

However, given the imperfection of legislation regarding the implementation of concluded contracts, there is a risk that the effectiveness of the above measures will be no more than 10-15%.

To quickly resolve the problems of employing specialists with higher medical education, primarily in primary health care, it is advisable to go in two directions:

  1. Stop issuing documents to doctors who have completed primary specialization, starting in 2007, using the following technology: 1) issue to employers (according to concluded agreements) certified copies of documents for hiring specialists; 2) store the originals in the Krasnoyarsk State Medical Academy or in the healthcare and drug supply agency of the regional administration; 3) upon completion of the specialist’s obligations to fulfill the contract (after three years), issue original documents.

  2. To create a system of educational loans for applicants and students to finance training costs with the condition of repaying the loan in the form of a three-year period of work in a medical institution in accordance with the concluded agreement in the required specialty.

^ The main results of the work of Krasnoyarsk State Medical Academy to implement the tasks of the priority national project in the field of healthcare for 2006

The implementation of the priority national project in the field of healthcare in the Krasnoyarsk Territory (hereinafter referred to as the Project), as in other constituent entities of the Russian Federation, is carried out according to the following priorities: 1) development of primary health care; 2) development of preventive medical care; 3) additional medical examination of the population; 4) improving medical care for newborns and women during pregnancy and childbirth; 5) providing the population with high-tech medical care; 6) project management and information support.

KrasSMA specialists are part of the headquarters for the implementation of a priority national project in the field of healthcare in the Krasnoyarsk Territory.

In accordance with the decision of the expanded board of the Ministry of Health and Social Development of Russia dated October 14, 2005 “On the tasks of implementing a priority national project in the field of healthcare,” KrasSMA, like other medical universities, is entrusted with the task of additional training, retraining and improving the professional level of specialists.

^ Organizational events

As part of the project implementation, the following organizational measures were carried out at KrasSMA: 1) changes and additions were made to the educational and production plan for advanced training and professional retraining of specialists for 2006; 2) mobile teams have been formed to train primary care doctors; 3) a working group has been created to coordinate activities for staffing primary health care.

A work plan for the Krasnoyarsk State Medical Academy for the implementation of the national project for the period until 2008 was developed and approved.

Agreements on cooperation in the field of training personnel for primary healthcare have been concluded between the Krasnoyarsk State Medical Academy and the Agency for Healthcare and Drug Supply of the Administration of the Krasnoyarsk Territory; Health Department of the Taimyr Autonomous Okrug; Health Department of the Evenki Autonomous Okrug.

Taking into account the main directions of implementation of the priority national project in the field of healthcare, an Action Plan for the development of postgraduate education for 2006-2008 was developed and approved.

The academic laboratory “Family Health” has been created and is functioning (order of the State Educational Institution of Higher Professional Education of KrasSMA Roszdrav dated March 17, 2006 No. 10-org).

^ Training (retraining), advanced training for general (family) practitioners, local internists and pediatricians

For 2006, within the framework of the training and production plan for advanced training for 2006 (taking into account its changes and additions), as well as at the expense of funds from the priority national project in the field of healthcare, underwent advanced training (professional retraining) 741 specialists primary health care(342 local therapists, 329 local pediatricians, 70 primary care physicians) of medical institutions in the Krasnoyarsk Territory, Taimyr and Evenki Autonomous Okrugs, the Republic of Tyva, the Republic of Khakassia.

In the process of advanced training (professional retraining), issues of healthcare organization are covered by the teaching staff of the departments of public health and healthcare; health insurance, management and marketing; health economics and medical law; issues of HIV infection and immunoprophylaxis - together with specialists from the Regional Center for the Fight against AIDS and Infectious Diseases.

A definite innovation was the preparation of handouts for cadets, including legislative and regulatory documents relating to the activities of the local therapist, local pediatrician, general practitioner, as well as regulatory and legal acts on the priority national project in the field of healthcare.

As part of professional development cycles, meetings between general practitioners, pediatricians, and obstetricians and gynecologists with the leadership of the healthcare and drug supply agency of the Krasnoyarsk Territory Administration and the Krasnoyarsk Regional Compulsory Health Insurance Fund have become a tradition.

During on-site training cycles, KrasSMA specialists provide organizational, methodological and practical assistance to practical healthcare.

Together with the Agency for Healthcare and Drug Supply of the Regional Administration, work was carried out to create a training base for the training of general practitioners in the Berezovsky district of the Krasnoyarsk Territory.

This year, special attention was paid to the recruitment of specialists for postgraduate forms of education (internship, clinical residency). Admission was carried out only on the basis of contracts for subsequent employment with priority for primary health care and for high-tech types of medical care - training of specialists for the Cardiovascular Surgery Center under construction in Krasnoyarsk (surgeons, cardiologists, transfusiologists, etc.).

^ Additional medical examination of the working population

To ensure the additional medical examination of the population Krasnoyarsk Territory, working in state and municipal institutions in the field of education, healthcare, social protection, culture, physical culture and sports and in research institutions in 2006, in the State Educational Institution of Higher Professional Education of KrasSMA Roszdrav in June of this year a cycle of thematic improvement was organized and carried out “ X-ray diagnostics of breast diseases” (9 specialists trained); in September-October - “Urology (for surgeons involved in medical examination)” (31 specialists), in November - thematic improvement cycles “Topical issues of endocrinology” (18 specialists), “X-ray diagnosis of breast diseases” (for specialists of the Republic of Khakassia).

During the year, the scientific direction “Health of the Healthy” was formed with a funding volume of up to 1.0 million rubles from the funds of the Krasnoyarsk State Medical Academy.

As part of the implementation of this area of the Project, additional medical examinations were organized and conducted for employees of Krasnoyarsk State Medical Academy together with the Municipal Health Institution “City Clinic No. 14” of Krasnoyarsk

^ Project management and information support

Much attention was paid information support of the Project. Since January 2006, monthly seminars and round tables have been held with interns and clinical residents in the above-mentioned specialties, where issues of implementing a priority national project in the healthcare sector, including improving the work and strengthening primary healthcare, are discussed.

In October of this year, seminars were held with 6th-year students of the pediatric and medical faculties, 5th-year students of the Institute of Dentistry on the Project in the Krasnoyarsk Territory and upcoming employment upon graduation from a higher educational institution. Future graduates are offered jobs in primary healthcare.

Krasnoyarsk State Medical Academy implements the objectives of the Project in close cooperation with state and municipal executive authorities. Thus, on September 2, 2006, KrasGM specialists took part in a meeting of the association of heads of local self-government “East”, which took place in Kansk under the chairmanship of the Governor of the Krasnoyarsk Territory A.G. Khloponin. During the meeting, special attention was paid to the development of the “institute” of general practitioners in the region, the formation of an inter-district medical center on the basis of the Kansk Central City Hospital.

Together with the Administration of the Federal State Employment Service for the Krasnoyarsk Territory, the Agency for Healthcare and Drug Supply of the Administration of the Krasnoyarsk Territory, a round table was held on September 26 on the topic “The labor market in the healthcare sector in the implementation of the priority national project “Health”. The problems of staffing specialists with higher and secondary medical education in municipal healthcare institutions of the region, the goals and objectives of the employment service in the implementation of the national project “Health”, and the experience of Krasnoyarsk State Medical Academy in training primary healthcare personnel were discussed.

As part of the advanced training cycle “Public Health and Healthcare” at the Berezovskaya Central District Hospital (Krasnoyarsk Territory), a round table was also held on the topic “National Project “Health” in the Krasnoyarsk Territory” with the participation of the Deputy Head of the Berezovsky District of the Krasnoyarsk Territory (Manakova T. A.), at which issues of organizing additional medical examination and immunization were discussed; providing primary medical and obstetric-gynecological care to the population of the suburban region of the Krasnoyarsk Territory.

In October of this year, Krasnoyarsk State Medical Academy specialists took part in on-site meetings (Lesosibirsk, Krasnoyarsk Territory) of deputies of the Legislative Assembly of the Krasnoyarsk Territory on the issue of “Development of healthcare in Lesosibirsk”. During the meeting, it was decided to hold a presentation of graduates for subsequent employment in Lesosibirsk in December 2006.

In November 2006, similar on-site meetings of deputies of the Legislative Assembly of the Krasnoyarsk Territory with the participation of Krasnoyarsk State Medical Academy specialists were held in the Irbeysky and Sayansky districts of the Krasnoyarsk Territory.

On October 26, 2006, with the participation of KrasSMA specialists, issues of implementing a priority national project in the field of healthcare were discussed at the Coordination Council for Postgraduate and Postgraduate Education of Healthcare Specialists of the Siberian Federal District, which took place in Tomsk.

As part of the implementation of one of the main activities of Krasnoyarsk State Medical Academy: “Improving the system of advanced training (professional retraining) of specialists with medical education, focused on the regional characteristics of the Krasnoyarsk Territory” in October 2006, at a meeting of the health commission of the Legislative Assembly of the Krasnoyarsk Territory, the issue of additional funding was considered (at the expense of the constituent entities of the Russian Federation) advanced training (professional retraining) of specialists for 2007 in the amount of 10.0 million rubles.

In general, it should be noted that all points of the decision of the Academic Council of the State Educational Institution of Higher Professional Education of the Krasnodar State Medical Academy of the Russian Federation of Health dated December 21, 2005 on the issue “On the national priority project “Health” have been fulfilled.

Thus, in 2006, the Krasnoyarsk State Medical Academy, within its competence, actively implemented the tasks of a priority national project in the field of healthcare.

^ Main directions for improving postgraduate education for 2007

In 2007, the implementation of the “Action Plan for the Development of Postgraduate Education for 2006-2008” and the “Work Plan of the Krasnoyarsk State Medical Academy for the Implementation of a Priority National Project for the Period until 2008” will be continued.

The plan for advanced training and professional retraining of specialists for 2008 will be formed on the basis of the “Register of Medical Workers” database with a focus on the “budget-extrabudget” technology, general improvement cycles of 144 hours and professional retraining cycles of more than 500 hours.

Additional training for primary care doctors (local therapists, local pediatricians, general practitioners) will continue, including under government contracts concluded as a result of an open competition for this work held by Roszdrav.

In 2007, it is necessary to improve the quality management system of postgraduate education.

During 2007-2008, the technology of on-site training cycles will be improved - providing not only training, but also comprehensive organizational and professional assistance to practical healthcare.

The development of paid educational services will continue.

Thus, as part of improving the personnel training system, by 2009 it is planned to create a competitive system of postgraduate education based on a unified policy in the field of education, taking into account the regional component, and the provision of medical care in the region.

Thus, the main results of the work are as follows:

  1. 428 people trained in internship

  2. 127 people trained in clinical internship

  3. 4,525 people underwent advanced training (professional retraining)

  4. An assessment of the employment of young specialists was carried out

  5. The KrasSMA website operates a notification system about vacancies in healthcare institutions of the city and region

  6. The objectives of the priority national project in the field of healthcare have been fully implemented

  7. An action plan for the development of postgraduate education until 2008 has been developed.

For the further development of a competitive system of postgraduate education on the basis of a unified education policy, taking into account the regional component, as well as the implementation of the tasks of the priority national project in the field of healthcare in 2007, the following tasks are planned:

  1. Fulfillment of the state task for the training of primary care doctors

  2. Improving the quality management system of postgraduate education

  3. Improving the technology of conducting on-site cycles

In 1999, the faculty celebrated its 38th anniversary. Its origins were advanced training and specialization courses for doctors, opened in 1951, which in 1961 received the status of a separate faculty. For 12 years (from 1961 to 1972) it was permanently headed by Candidate of Medical Sciences, Associate Professor V.V. Chernyavsky. During this period, professors Yu. M. Lubensky, B. V. Rapoport, associate professors I. V. Krasitsky, A. N. Protopopova, M. S. Zyryanova, V. I. Lazarenko and others taught at the faculty.

The goal of the faculty since its creation remains the continuous training of highly qualified specialists for practical healthcare in the Krasnoyarsk Territory and its neighboring regions. The main areas of work of the FPC and teaching staff remain the general and thematic improvement of doctors, their specialization, professional retraining, and since 1996, certification of doctors.

The faculty trains specialists in 11 departments and 11 courses. The faculty provides training in 28 medical specialties: obstetrics and gynecology, neurosurgery, surgery, endoscopy, therapeutic dentistry, maxillofacial surgery, radiology, dermatovenerology, traumatology and orthopedics, emergency medicine, neuropathology, functional diagnostics, ophthalmology, physiotherapy, immunology, general medical practice, neonatology, psychotherapy, etc.

Teams of departments and courses conduct visiting cycles in cities and regions of the region, Khakassia, Tuva, Tomsk, Kemerovo, Sakhalin and other regions. Up to 2,500 people undergo specialization and improvement annually, and more than 30 certification cycles are carried out.

The faculty successfully employs 101 teachers, including 20 Doctors of Science and 52 Candidates of Science. The degree of composure of faculty members exceeds 70%. The scientific and pedagogical composition of the team meets the highest requirements of educational programs.

The faculty trains 44 clinical residents and 24 graduate students. Under the guidance of professors, 16 applicant doctors are completing their dissertations.

Most of the departments and courses of the faculty are located in the largest multidisciplinary hospitals in Krasnoyarsk, using modern technologies and equipment.

Over the past 5 years (1994–1999), the faculty has prepared 23 educational and methodological developments, 87 teaching aids, published 11 monographs, 12 collections of methodological and scientific works, 3 textbooks.

The traditions of the faculty are supported by its veterans - professors F. F. Kostyuk, N. S. Dralyuk, L. S. Grakova, N. N. Nikolaeva, associate professors A. B. Grinshtein, V. E. Parilov, N. I. Davydova and other.

The faculty is developing and looking confidently into the future. The administration and administration of the Medical Academy pay great attention to the work of the faculty, rightfully considering it one of the leading faculties of the academy.

Department of Anesthesiology and Reanimatology No. 1

In the early 60s, a new medical science and specialty began to intensively develop in our country - anesthesiology and resuscitation (A&R). On the initiative of Professor I. I. Shafer, in 1967, a course in teaching A&P to 2nd and 3rd year students was introduced at KrasSMA at the Department of General Surgery. Anesthesiologist-resuscitator assistant I.P. Nazarov was added to the staff of the department. From 1967 to 1974, teaching was conducted on the basis of City Hospital No. 1, from 1975 to 1980 - in the newly opened State Emergency Hospital, and from 1980 to 1987 - in the road hospital of the station. Krasnoyarsk and Medical Unit No. 7. Over these 20 years, about 15 thousand students have been trained in the A&R course. All these years the teacher was taught by assistant I.P. Nazarov, who at the same time headed the departments of A&R and conducted active medical and scientific work. In 1970, he defended his candidate’s thesis “The state of acid-base balance in children under various types of anesthesia”, and in 1984 - his doctorate on the topic “Long-term anti-stress therapy with gangliolytics in the preoperative period during and after surgery.” In January 1986, he was elected by competition as a professor of the Department of General Surgery, and in July 1997 he was awarded a diploma of an academician of MANEB.

In 1986, at the Department of General Surgery, simultaneously with teaching A&R to students, a 5-month cycle of specialization for doctors was introduced. Professor I.P. Nazarov was appointed responsible for this cycle. In 1987, the A&R course was transformed into an independent department of anesthesiology and resuscitation of the Faculty of Training and Teaching Staff.

The department is fully staffed. The department is headed by Doctor of Medical Sciences, Professor, Academician of MANEB I.P. Nazarov with 32 years of teaching experience and 37 years of practical work experience in the specialty. The team includes associate professor E.V. Voloshenko and 5 assistants (candidate of medical sciences E.V. Pugonin, candidate of medical sciences D.V. Ostrovsky, E.O. Vasilyeva, S.V. Kashcheev, E. . V. Ilchenko). The employees are experienced specialists with practical experience from 10 to 25 years, teaching experience - from 5 to 13 years.

The department was one of the first in Russia to begin training interns in A&R at medical institutes. Classes in anesthesia and intensive care are also conducted in the cycles of obstetrics and gynecology, traumatology and surgery according to agreed programs.

22 methodological recommendations were published on topical issues of A&R. A computer version of the “Expert System for the Treatment of Burn Shock” has been created on the basis of burn resuscitation. Employees of the department supervise the training of intern doctors at the bases of the Krasnoyarsk Territory, Khakassia and Tuva.

The department's staff is actively carrying out research work, which began in the A&R course back in the seventies. The main direction of these studies is the topic “Long-term anti-stress therapy in surgical patients.” Within the framework of the scientific topics of the department over the years, more than 400 articles have been written, 7 monographs have been published and are in print, 5 patents for inventions, 82 rationalization proposals have been approved. 2 doctoral dissertations were completed (I. P. Nazarov, 1983; A. A. Popov, 1996), 14 candidate dissertations.

In recent years, the staff of the department have paid especially much attention to the development of the topic of stress-protective and adaptogenic therapy, extracorporeal methods of detoxification and immunocorrection in patients with severe burns. Many methods of anti-stress protection and intensive therapy developed at the department are successfully used in medical institutions of the city, region and country. Recognition of the practical and theoretical significance of the methods was the proposal of the organizing committees of many congresses and conferences (in Omsk, Krasnoyarsk, Moscow, Nizhny Novgorod, Japan, etc.) to give lectures on the use of stress protectors in surgery and cambustiology. The lectures were given by I.P. Nazarov and aroused great interest in Russia and foreign countries.

It can be stated with confidence that the department successfully carries out all sections of the work assigned to it.

Department of Anesthesiology and Reanimatology No. 2

Since 1990, a new stage in the teaching and development of anesthesiology and resuscitation began, when the Department of Emergency Care and Intensive Care of Children of the Faculty of Pedagogics and Teaching Staff was created (since 1996 - Department of Anesthesiology and Reanimatology No. 2 of the Faculty of Education and Training, Head - Professor A.P. . Kolesnichenko) in connection with the needs of practical healthcare.

Since the organization of the department, two main goals have been set for it: to improve the training of students of the pediatric faculty and pediatricians, anesthesiologists and resuscitators in providing emergency care and intensive care to children with critical conditions of various origins; develop and implement a number of organizational, treatment and methodological approaches to reducing child mortality in the region.

The last years of teaching anesthesiology and resuscitation for the specialty “Pediatrics” for 6th year students are devoted to the study of modern general principles of pain management and etiopathogenetic approaches to intensive care in children in critical condition. The learning process is undoubtedly facilitated by the fruitful cooperation of the Department of Anesthesiology and Reanimatology No. 2 of the Faculty of Pedagogical Education and Teaching Staff with the departments of anesthesiology and resuscitation of the city clinical hospital No. 20 and the regional children's hospital No. 1. Thanks to this, students have an excellent opportunity to master theoretical and practical material in all areas of pediatric anesthesiology and resuscitation.

The department carries out a lot of methodological work. Methodological developments for teachers, classroom and extracurricular work of cadets, and test control have been created on all topics.

Since the organization of the department, 6th year students of the Faculty of Pediatrics have been studying in parallel with the cadets.

Employees of the department under the leadership of A.P. Kolesnichenko implemented a new conceptual position on the end-to-end training of pediatricians in critical care medicine of childhood together with other departments - pediatrics, pediatric surgery, anesthesiology and resuscitation No. 1 of the Faculty of Training and Teaching Staff.

In 1990–1993 in the region, thanks to the efforts of the department staff and practical healthcare, a long-term program to reduce infant mortality was developed and implemented.

For the first time in Krasnoyarsk, a regional center for pediatric anesthesia, resuscitation and intensive care was opened on the basis of the regional hospital, in the creation of which a doctor of the highest category, I. N. Popandopulo, provided great assistance. Since its opening, this medical unit has become the main base of the department for conducting the educational and therapeutic process.

Then, on the basis of the center, a resuscitation and consultation center (RCC) was deployed for older children, and subsequently for newborns.

For the regions of the region and Krasnoyarsk, the threat-based advisory-automated system (UCAS), developed by St. Petersburg and Yekaterinburg specialists, was adapted and implemented.

At the same time, computer programs DINAR-1 and DINAR-2.11 were introduced, which made it possible to place all children in the region with threatening conditions under dynamic intensive observation and provide preliminary syndromic diagnosis of critical conditions.

The department prepared and defended 1 candidate’s dissertation on the topic “Selection of optimal parameters and modes of mechanical ventilation in children depending on the type of acute respiratory failure.”

The main directions of scientific and methodological activities of the department:

  • problems of respiratory support in patients with critical conditions;
  • problems of correction of endotoxicosis in patients with critical conditions;
  • changes in hemostasis in patients with critical conditions;
  • choice of pain management methods in an obstetric clinic;
  • improving the organization of the structure of anesthesiology and intensive care services in order to reduce child and maternal mortality.

Department of Surgical Diseases

In 1979, a new course of surgical diseases was created at Krasnoyarsk State Medical Academy to improve the qualifications of doctors, which gradually grew into the department of advanced training for doctors.

The organizer and head of this department was Doctor of Medical Sciences, Professor O. E. Nifantiev. Teaching was conducted at the Krasnoyarsk station road hospital and the city emergency hospital. Basically, the department was engaged in the general improvement of doctors in surgery through civil defense on a 4-month cycle.

In 1986, the department moved to the base of the city clinical hospital No. 20. The personnel composition of the department changed; the different profile of the clinical base of hospital No. 20 with many specialized departments made it possible to change the topic of teaching. New cycles are appearing: “Proctology”, “Selected issues of surgery for internship supervisors”, “diagnosis and emergency care for acute diseases and injuries of the abdominal cavity and urinary tract”. A new cycle of professional training for teachers of medical schools is being introduced, which lasted until 1992.

During this period, the department was engaged in extensive research work on the problems of peritonitis. A new method for sanitation of the abdominal cavity and its drainage has been developed, clinical and laboratory criteria for assessing intraoperative measures and indications for performing programmed relaparotomies have been determined. A fundamentally new device for the express preparation of sanitation solutions “Geyser” has been developed and put into mass production, which has found wide use in emergency surgery.

Under the leadership of O. E. Nifantiev, 1 doctoral and 9 candidate dissertations were completed and defended.

Since 1993, the department has been headed by Doctor of Medical Sciences, Professor A. M. Sukhorukov.

The clinical base of city hospital No. 20 has become more specialized: a vascular surgery department has appeared, the proctology department has grown into a regional coloproctology center, and the endoscopic service has grown significantly. The department received another clinical base - the surgical department in Medical Unit No. 96. The staff of the department has changed and been replenished.

All this made it possible to significantly change the thematic plan of classes with cadets, taking into account the requests and needs of regional healthcare. New cycles have appeared: “Emergency surgery of the thoracic and abdominal cavities”, “Surgery of the liver, biliary tract and pancreas”, “Selected issues of angiology”, “Current issues of endoscopy in the diagnosis and treatment of diseases of the abdominal and thoracic cavities”. A lot of work has been done to create programs for these cycles and teaching aids.

Since 1998, new 3-month certification cycles in surgery and endoscopy have been conducted.

Since 1999, the department has additionally received an emergency care course, headed by Doctor of Medical Sciences, Associate Professor A. A. Popov.

The department is the base where internship training in surgery takes place annually.

The subject of scientific research at the department remains emergency conditions and purulent-inflammatory diseases of the thoracic and abdominal cavities, angiology and coloproctology. The department carries out 1 doctoral and 3 candidate dissertations.

One of the oldest employees of the department is Associate Professor N. I. Davydova. The team also includes associate professor V. A. Fokin, assistants, candidates of medical sciences A. E. Popov, V. Department of Clinical Laboratory Diagnostics

In 1983, the Department of Specialization and Improvement of Laboratory Technicians was created at KrasSMI, which in 1985 was renamed the Department of Clinical Laboratory Diagnostics, which was evidence of the recognition of clinical laboratory diagnostics as an independent medical discipline.

The main base of the department all these years has been the clinical diagnostic laboratory of the regional clinical hospital No. 1. Part of the classes are conducted on the basis of the leading clinical diagnostic laboratories of the city (regional children's hospital, AIDS center, medical genetic center, city hospital No. 4, city anti-tuberculosis dispensary and etc.).

From the opening day to the present day, the department has been headed by Doctor of Medical Sciences, Professor Yu. O. Toroschin. The department employs 2 professors, 2 associate professors, 2 assistant candidates of medical sciences.

During its work, the department has trained a whole group of highly qualified specialists who work both at the department and in medical institutions of the city: they defended their doctoral dissertations and became professors Yu. O. Toroschin and T. A. Kozhevnikova, received the title of associate professor T. P. Tsygankova, T. I. Udovitsina, V. T. Murnikov, I. A. Olkhovsky, defended his Ph.D. thesis by I. A. Zaliznyak. The large clinical diagnostic laboratories of the city were headed by former employees of the department D. A. Grishchenko - the chief specialist in laboratory diagnostics of the region, V. M. Alferov, I. A. Olkhovsky - the head of the clinical diagnostic laboratory of the regional AIDS center.

In just 15 years, the department has conducted 88 cycles (of which 42 over the last 5 years) and trained 13,700 cadets (over the last 5 years - 699 people) from Krasnoyarsk, the Krasnoyarsk Territory, other regions and territories of most territories of the former USSR and Russia ( Ufa, Murmansk, Tyumen, Perm, Khabarovsk, Vladivostok, Sakhalin, Kamchatka, Buryatia, Altai Territory, Kemerovo, etc.).

The qualitative composition of the cadets is heterogeneous: in our specialty there are doctors from the medical and pediatric faculties, doctors from the medical and biological faculty, biologists who graduated from the university, teachers of biology, chemistry, physics, sanitary doctors, pharmacists, physicists, chemists, biophysicists and even technical workers.

During the educational process, computer control and training programs are used, as well as familiarization with modern laboratory equipment.

The department initiated the organization in the Krasnoyarsk Territory of the regional regional association of laboratory diagnostic workers, which is part of the Russian Association.

The department conducts scientific research in the field of hemostasis, clinical immunology, immunodiagnostics, deals with the problems of neuroimmunology, longevity and environmental immunogenetics. For many years, active scientific research has been carried out on the problem of atherosclerosis together with the Research Institute of Medical Problems of the North of the Siberian Branch of the Russian Academy of Medical Sciences. Currently, a new phenomenon in biology and medicine associated with the structuring of biological fluids is being studied. The work is being carried out jointly with the Institute of Gerontology of the Russian Academy of Medical Sciences and KSU, and developments are underway in the field of new laser technologies in medicine. As part of the integral program of the country's universities, the department actively participates in the training of medical physicists together with KSU. The most interesting scientific works at the department are related to the problems of psychogenetics. This scientific direction is new, still developing, but interesting results have already been obtained. Scientific research on psychogenetics is carried out jointly with the International Scandinavian University (Sweden, Stockholm), the Department of Medical Genetics of the International Institute of Peoples' Friendship, and the Russian Medical Genetics Center of the Russian Academy of Medical Sciences (Moscow).

The medical work of the department is based mainly in the hospitals of the regional clinical hospital, where the teaching staff provides consultations to patients and conducts advisory work among doctors.

In terms of further development of the department, it is planned to conduct cycles on management and marketing for health care organizers of the region at any level for medical institutions and laboratories of all forms of ownership. The department plans to conduct cycles for general practitioners in clinical laboratory diagnostics, narrow-profile cycles in hematology, clinical biochemistry, and laboratory diagnostics of emergency conditions. In the light of the concept of integration of departments - in medical genetic research.

Department of Neurosurgery and Neuropathology

The Department of Neurosurgery and Neuropathology for advanced training of doctors was organized in 1980. The creation of the department was caused by the need to improve neurosurgeons. In addition, there was an urgent need to improve the knowledge of surgeons, traumatologists, emergency physicians, and primary care physicians in matters of emergency neurosurgery and neuropathology, as well as to familiarize senior students with the basics of neurosurgery. However, the formation of the neurosurgical service, which later became the basis of the department, began long before its opening.

In 1969, an associate professor course in neurosurgery was created at the Department of Hospital Surgery for classes with 5th and 6th year students.

N. S. Dralyuk was elected associate professor. During these same years, classes were held with neuropathologists at the department of nervous diseases (head of the department is Professor V. A. Rudnev, associate professor A. B. Grinshtein). Subsequently, the department was formed from the merger of these two cycles.

Since the establishment of the department, its bases have been the regional clinical hospital No. 1 and the emergency hospital with a total number of beds of 240. The neurosurgical operating room of the Clinical Clinical Hospital No. 1 is equipped with modern equipment that allows microsurgical operations for tumors and aneurysms of the brain. There is all the necessary equipment for conducting examinations at the modern level (MRI, CT, angiography, etc.).

From 1991 to the present, the department is headed by a full member of the New York Academy of Sciences, Doctor of Medical Sciences, Professor M. G. Draluk. Teaching is conducted by 3 doctors of medical sciences, 2 associate professors, 3 candidates of sciences.

The employees of the department carry out a large amount of scientific research in various sections of neurosurgery and neuropathology: monographs have been published, chapters have been written in various manuals, methodological manuals for practitioners, more than 300 works have been published in various collections and journals, including foreign ones. A conference of neurosurgeons of the Russian Federation, a plenum of the board of the society of neurosurgeons, and a Russian-Japanese conference of neurosurgeons were held. For the first time in our country, long-term intracarotid infusion was developed and introduced for the treatment of severe inflammatory and vascular diseases of the brain (Professor N. S. Draluk, 1966). The study of the microvasculature of the brain stem in trauma, vascular diseases and brain tumors has been developed in detail and continues to be improved (Professor M. G. Draluk, 1980). The results of the research were presented at national and international conferences. He was also the first to study and apply transoperative cytological diagnosis of brain tumors (1979). Together with the Department of Neuropathology, a functional assessment of the restorative reserves of the brain in case of its organic lesions is carried out (Doctor of Medical Sciences A. A. Narodov, 1997). Long-term intra-aortic infusion for severe combined traumatic brain injuries has been developed and implemented (Professor M. G. Draluk, Ph.D. I. P. Grinev, 1997). Research is being conducted to study the clinical picture and diagnosis of vascular diseases of the brain and diseases of the peripheral nervous system (Associate Professor A. B. Grinshtein).

Close connections with practical healthcare made it possible to create a powerful neurosurgical service in the region, to increase the knowledge of primary care doctors in the areas of emergency neurosurgery and neuropathology. The department and clinic of neurosurgery continue to develop and improve in full collaboration with the country's leading institutions, with practical healthcare, and continue to strengthen international ties.
